identifying occupational-specific technical standards required of students in the program. Please print, read and Sign this document and include it in your application packet. If you have questions or concerns related to your ability to meet these described requirements, please contact your academic advisor.

If you completed pre-DM coursework at another institution, you will need to include a copy of the OFFICIAL transcript you submitted to BPCC Admission in your DM clinical application packet.
The 20 students selected into the clinical program each year will be required to adhere to additional Clinical Requirements.![]()
These requirements include providing additional documentation, completing additional immunizations/tests and following additional policies/procedures unique to Allied Health Clinical Programs. It is the responsibility of the selected student to (a) complete/fulfill the listed requirements and (b) cover the costs of those requirements. Students will be notified upon selection into clinicals which procedures to follow and what documentation to provide.
If you are employed in a foodservice at a healthcare facility with a Registered Dietitian, print and read the Student Acknowledgement of the Employer Waiver of Dietary Management Program Clinical Requirement.![]()

and ask your director of Human Resources to sign the form. Submit the signed form with your packet if your employer agrees. If submitted, you may omit the asterisked items on the Clinical Student Disclosure of Clinical Requirements list above.
