Application Packet Instructions
- Deadline for submission (hand-delivery or postmark date) – April 15, 2009
- The following documents should be completed, placed in a sealed single large envelope, and addressed to:

- Completed prerequisite course verification form
- Official Transcripts (not required if all pre-PTA coursework is from BPCC)
- Two Clinical Observation Rating Forms (CORFs) completed and signed by the PTs or PTAs with whom you completed the required 40 hours observation, volunteer or work experience. (See detailed instructions below)

Essential Requirements
The PTA Program has established Essential Requirements,identifying occupational-specific technical standards required of students in the program. Please print, read and Sign this document and include it in your application packet. If you have questions or concerns related to your ability to meet these described requirements, please contact your academic advisor.
Application Form
Neatly type all information requested on PTA Clinical Application Form.It must be printed, SIGNED, and included in your application packet.
ARF (Academic Rating Form) Completion Request Form
Part of your application packet is an academic rating form completed by one of your college instructors. Complete this ARF request formproviding us with the name and email address of the instructor you would like us to use. The two-page Academic Rating Formwill be emailed to the instructor and will be returned directly to the program admissions committee for scoring via email.
CORF (Clinical Observation Rating Form)
You are required to complete a minimum of 40 hours observation, volunteer or work experience in two different physical therapy settings (20 hours each) under the supervision of two different licensed PT/PTAs. If unfamiliar with PT facilities in the area, you may elect to contact a suggested PTA observation site to request an observation opportunity.
- Plan ahead and start early – These observation experiences should be scheduled to take place beginning in the fall semester and should be completed no later than March in the spring semester of the year in which you plan to apply.
- After your observation experience is complete, print one Clinical Observation Rating Formfor each PT/PTA you observed (2 forms minimum) and give them the form along with an ADDRESSED and STAMPED envelope.

Bossier City, LA 71111 - The PT/PTA should be asked to complete the form, seal it in the provided envelope, sign across the seal and mail directly to the college at the address above.
- It is the responsibility of the applicant to follow up with the PT or PTA completing the forms so that they are mailed with a postmark date of no later than the April 15th deadline.
Prerequisite Course Verification Form
Use the Prerequisite Course Verification Formto document each course you have taken (including when, where and the grade you earned for each attempt). Print and complete this form by reviewing and highlighting a student copy of your own transcript to make sure you have completed every required course. You must include every pre-PTA course attempted for which you earned a grade. In some instances (when it would improve your score) a higher level course may be substituted for a lower level prerequisite. Check with your advisor if you have questions about whether this applies to you. Print this form when completed and include it in your packet along with the highlighted student copy of your transcript.
Official Transcripts/Other Institutions
If you completed pre-PTA coursework at another institution, you will need to include an OFFICIAL transcript from that college in your PTA clinical application packet.
Clinical Student Disclosure of Clinical Requirements
The 20 students selected into the clinical program each year will be required to adhere to additional Clinical Requirements.These requirements include providing additional documentation, completing additional immunizations/tests and following additional policies/procedures unique to Allied Health Clinical Programs. It is the responsibility of the selected student to (a) complete/fulfill the listed requirements and (b) cover the costs of those requirements. Students will be notified upon selection into clinicals which procedures to follow and what documentation to provide.
