The PTA Program has established Essential Requirements,
identifying occupational-specific technical standards required of students in the program. Please print, read and Sign this document and include it in your application packet. If you have questions or concerns related to your ability to meet these described requirements, please contact your academic advisor.

It must be printed, SIGNED, and included in your application packet.
Following the instructions on the Employee Rating Form
, you must print and provide this form to one current or former employer. The employer is to complete the form, evaluating your job performance while under his/her supervision, and return it by mail to the PTA Program Admissions Committee. The form must be received by the April 15th deadline in order for the applicant's packet to be considered. It is the responsibility of the applicant to follow up with the employer and Program to verify that the form has been sent/received.
You are required to complete a minimum of 40 hours observation, volunteer or work experience in two different physical therapy settings (20 hours each) under the supervision of two different licensed PT/PTAs. If unfamiliar with PT facilities in the area, you may elect to contact a suggested PTA observation site to request an observation opportunity.
Use the Prerequisite Course Verification Form
to document each course you have taken (including when, where and the grade you earned for each attempt). The applicant must pay careful attention to and follow explicitly the directions for completion/submission of this document outlined on the bottom of the form. Failure to complete/submit the form following those instructions may result in disqualification of the application packet.
Transfer students must complete all BPCC College Admissions forms and requests for official transcripts
to be sent to the BPCC Admissions office early in the spring semester so that all transfer courses are accessible in the BPCC system (are visible on the BPCC transcript) by the April 15th application deadline.
The 20 students selected into the clinical program each year will be required to adhere to additional Clinical Requirements.
These requirements include providing additional documentation, completing additional immunizations/tests and following additional policies/procedures unique to Allied Health Clinical Programs. It is the responsibility of the selected student to (a) complete/fulfill the listed requirements and (b) cover the costs of those requirements. Students will be notified upon selection into clinicals which procedures to follow and what documentation to provide.
