Application Packet Instructions

  • Deadline for submission (hand-delivery or postmark date) – April 15, 2013
  • The following documents should be completed, placed in a sealed single large envelope, and addressed to:

Academic Rating Request Form

All ALHT 109 instructors at BPCC complete and archive Academic Rating Forms at the end of each semester for each student in the class. This rating form is an evaluation, by the course instructor, of the student in the areas of attitude, time management, attendance, critical thinking, integrity, quality of work, and oral communication.

The Academic Rating Request Form you are required to complete and submit as part of your application packet is your documentation of the instructor and semester in which you completed ALHT 109 at BPCC. Following the instructions on the Academic Rating Request Form,PDF File print, complete and submit this form to the division secretary, Ms. Tonia Sharp, early in the spring semester, so that the Academic Rating Form completed by your ALHT 109 instructor may be retrieved and received by the selection committee by the April 15 deadline.

If you took ALHT 109 prior to the FALL OF 2012, you must contact a PTA Program advisor to request that an alternate college instructor (for a course you took fall 2012 or are enrolled in for spring 2013) be utilized for completion of an Academic Rating Form.

If you are planning to take ALHT 109 in the summer “B” (June) session of 2013, you must contact a PTA Program advisor to request that the deadline for completion of this form be extended until registration is complete for that summer ALHT 109 course.

CORF (Clinical Observation Rating Form)

You are required to complete a minimum of 40 hours observation, volunteer or work experience in two different physical therapy settings (20 hours each) under the supervision of two different licensed PT/PTAs. If unfamiliar with PT facilities in the area, you may elect to contact a suggested PTA observation site to request an observation opportunity.

  • Plan ahead and start early – These observation experiences should be scheduled to take place beginning in the fall semester and should be completed no later than March in the spring semester of the year in which you plan to apply.
  • After each observation is complete, print one Clinical Observation Rating Form for each PT/PTA you observed (2 forms minimum) and give them the form along with an ADDRESSED and STAMPED envelope.
  • You should address the envelope to:

  • The PT/PTA should be asked to complete the form (including providing contact information on the form for verification of scores), seal it in the provided envelope, sign across the seal and mail directly to the college at the address above.
  • Each clinician (PT or PTA) completing your form must have observed/worked with you a minimum of 20 hours (forms reflecting fewer than 20 hours will not be accepted).
  • Observation forms completed by a clinician who is a family member of the applicant will not be accepted.
  • It is the responsibility of the applicant to follow up with the PT or PTA completing the forms so that they are mailed with a postmark date of no later than the April 15th deadline.

Prerequisite Course Verification Form

The applicant must pay careful attention to and follow explicitly the directions for completion/submission of this document outlined on the bottom of the form. Failure to complete/submit the form following those instructions may result in disqualification of the application packet.

BPCC College Admissions/Official Transcripts/Other Institutions

Transfer students must complete all BPCC College Admissions forms and requests for official transcriptsPDF File to be sent to the BPCC Admissions office early in the spring semester. All transfer courses must be accessible in the BPCC system (be visible on the BPCC transcript) by the April 15th application deadline in order for an application packet to be accepted.

Clinical Student Disclosure of Clinical Requirements

The 20 students selected into the clinical program each year will be required to adhere to additional Clinical Requirements.PDF FileThese requirements include providing additional documentation, completing additional immunizations/tests and following additional policies/procedures unique to Allied Health Clinical Programs. It is the responsibility of the selected student to (a) complete/fulfill the listed requirements and (b) cover the costs of those requirements. Students will be notified upon selection into clinicals which procedures to follow and what documentation to provide.
