VA Form 22-1990
Application for Veterans Educational Benefits, required of first time veteran benefit recipients who have served on active duty or who are actively serving in a reserve component. Only one application is required unless the student is applying for a different benefit.

Request for Change of Program or Place of Training, required for all prior or all transfer students who are not in the same degree they were most recently certified.

Veteran Application for Veterans Educational Benefits, required of first time dependents of service members who were disabled or died as a result of active duty service.

Dependent Request for Change of Program or Place of Training, required for all prior or transfer students who are not in the same degree they were most recently certified.
