Proof of IDENTITY

• Family Educational Rights and Privacy Act (FERPA) - The Family Educational Rights and Privacy Act (FERPA) is required to receive access to any educational record (Financial Aid, Advising, and Admissions). If you have never used Dynamic Forms before you will need to create an account. This account is separate from your LOLA information and is an username and password of your own creation.
