Admission Forms
Louisiana law requires immunizations against measles, mumps, rubella, whooping cough (Pertussis), Meningococcal Disease (Meningitis), Haemophilus Influenzae B (HIB), Poliomyelitis, and tetanus-diphtheria for all first-time Louisiana Community and Technical College Systems students born after 1956.
Steps to submit Student Immunizations to BPCC:
- Login to your LoLA account.
- Locate the section titled Academic Links.
- Select Required Admissions Forms.
All male U.S. citizens, regardless of where they live, and male immigrants, whether documented or undocumented, residing in the United States, who are 18 through 25, are required to register with Selective Service. If you are 26 years or older and have not registered for the selective service, then you must request a STATUS INFORMATION LETTER.
Steps to submit Selective Service to BPCC:
- Login to your LoLA account.
- Locate the section titled Academic Links.
- Select Required Admissions Forms.
Steps to Submit a Change of Major Request
- Access LoLA.
- Locate the section titled "Academic Links".
- Select Link “Change of Program/Major”.
- Select “Change This” under the major box.
- Select Major and Campus, if applicable.
- Select Accept.
- Select Submit Changes.
- Select Confirm.
- If a student wants to cancel the request and choose a different major, they will select "Revert to Original."
To obtain an official Louisiana GED transcript, you can order it for $15. If you have questions about receiving a copy of your GED, HiSET, or High School Equivalency, please call 225-922-2800.
You may request your official transcript online with a debit or credit card. Official transcripts cost $10.00 each. When prompted for “Recipient Email”, enter the receiving school email address, not your email address.
The Family Educational Rights and Privacy Act (FERPA) is required to receive access to any educational record, including those from Financial Aid, Advising, and Admissions.
If this is your first time using Dynamic Forms, you will need to create a new account. This account is separate from your LoLA account, and you will create your own username and password specifically for Dynamic Forms.
Once your FERPA form is submitted, it will be reviewed and processed within 1–3 business days. After it’s processed, the FERPA hold will be removed from your account.
To help avoid delays, please do not submit multiple copies of the form. During peak registration periods, processing may take a little longer due to high submission volumes, but rest assured—we’re working hard to review every form as quickly and carefully as possible.
Important: Be sure to write down and securely store your FERPA access code in a memorable and safe location. Several departments may require this code to verify your identity before sharing any educational information. Having it easily accessible will help ensure faster service and prevent unnecessary delays.

The Louisiana State Services and Benefits Legal Status and Accountability Act (Act 419), promotes transparency and accountability in state services provided to individuals without lawful immigration status and unaccompanied alien children. It applies to all state agencies and institutions that extend such services, including all Louisiana higher education institutions. Louisiana colleges must verify immigration status, track services rendered, and report these outcomes annually.
Students who identify as non-citizens in their admission applications must submit additional documentation. Accepted documentation includes:
- Original unexpired foreign passport with valid visa stamp
- Permanent Resident card
- I-94 Arrival/Departure Record (obtainable at the official CBP site)
- Unexpired USCIS nonimmigrant petition approval notices
- Form DS-2019 (Certificate of Eligibility for Exchange Visitor, J-1)
- Form I-20 (Certificate of Eligibility for Nonimmigrant Student)
- Employment Authorization Documents
- Verification via the federal SAVE system or equivalent DHS too

- Academic Renewal Application
- Bossier Parish Community College provides students who have not been enrolled in college due to academic deficiencies the opportunity to renew their academic record. The student must not have been enrolled in college level course work for one year (12 months), demonstrate that the conditions that led to the academic deficiencies have changed, and complete the necessary steps to be considered for Academic Renewal. Academic Renewal can only be awarded once in an academic lifetime at any LCTCS college and cannot be declared for any period that was previously used for an awarded credential. - Directory Information Opt Out Form
- The “Opt-Out for Directory Information” form is for students that explains how the college may share certain personal information about you — name, address, email, photo, major, enrollment status, etc. — under federal privacy law Family Educational Rights and Privacy Act (FERPA). If you do not want this information released, you should fill out and return the form to the Admissions/Registrar’s Office by the end of the second week of class. Filling it out prevents the college from releasing your “directory information” to outside groups. Please reach out to admissions@bpcc.edu to receive this application.
- Course Audit Form
- This form is used to request permission to audit a class. Auditing allows you to attend the course without earning credit or receiving a grade. Simply provide your information, list the course you wish to audit, and sign the form. You are still expected to follow attendance requirements and pay standard tuition and fees. Once processed, the course will appear on your record with an AU designation. Please reach out to admissions@bpcc.edu to receive this application.
